The Lead Staff/CMT is responsible for the oversight of their assigned home(s), the supervision of the house staff, providing direct support to individuals within the house, and the coordination of services of the individuals. The Lead Staff will be responsible for ensuring proper staffing of shifts and for managing the operations of the assigned residence. The incumbent will also participate in training and supervising CMT’s.

This position will require the following:
- Minimum of two (2) years of documented experience working with individuals with developmental or intellectual disabilities
- One (1) year of supervisory experience working with the population served.
- Good interpersonal skills and the ability to communicate and work effectively, individually and as part of a team with subordinates, other staff and supervisors throughout the organization.
- Acceptable criminal background check.
- CMT license.
- Driver’s License.
- High School Diploma required.
Other job duties:
- 24/7 On-call responsibilities, with mandatory weekly on-call coverage once every six weeks.
- Handle staff call outs and coverage and Cover shifts as needed with supervisor approval.
- Hold weekly team meetings and supervisions with staff that need additional support.
- Shadow/observe, coach and give feedback to CMT’s while visiting homes. This includes reviewing start/end of shift, administering medications, morning/evening routines, etc.
- Timely response to texts and requests from CMT’s and administrative office throughout the day.
- Purchase groceries and supplies for the house and submit receipts weekly.
- Review ICM daily for attendance, medication compliance, task completion, medication errors. et
- Verify medications in ICM.
- Work with meaningful day services program staff when individuals need to come home due to health or behavioral issues.
- May be required to drive clients to the community and to several places.
- Ensures that houses are kept clean and staff are completing all related, necessary paperwork including shift checklists.
- House Leader will support person in accordance with the Person-Centered Plan, Behavior Support Plan (if applicable), and Nursing Care Plan.
- Check and respond to text messages daily to maintain proper communication.
Available Shift:
- 3:00pm – 11:00pm preferable
- Monday through Friday (Week Days Shift) or Friday through Monday (Weekends Shift)

DUTIES & RESPONSIBILITIES:
- Review and verify that all Person-Centered Plans are accurate and properly implemented.
- Document outcomes, goals, and strategies as part of the Person-Centered Planning process, ensuring progress is being made toward achieving them. Documentation and preparing of (SIP, DSAT, PSS)
- The goal of achieving accreditation is to enhance person-centered service delivery and improve life outcomes for people living with intellectual and developmental disabilities.
- As a Quality Reviewer, you will be an active contributor in this exciting and impactful initiative.
- This is a field-based position which will require you travel regularly throughout the residential units. Your responsibilities will include:
- Meeting with individuals in service as well as their families and guardians,
- Meeting with people who work at residential, day service and CCS provider organizations
- Conducting provider reviews to determine alignment with person-centered principles and CMS regulations
- Data collection and report writing
- Participating in various quality improvement initiatives
- Partnering with the clients we serve to conduct PSP Surveys.

EDUCATION & EXPERIENCE:
- Bachelor’s degree in Social Work, Psychology, or a related field is required (Master’s degree preferred).
- Minimum of 5 years of experience working with individuals with developmental disabilities, including at least 3 years in a Program Manager role.
- Strong leadership abilities and experience working with individuals that have developmental disabilities, particularly within the DDA waiver programs.
- Excellent networking abilities and a proven capacity to lead a team effectively.
- Strong relationship management skills, with the ability to think strategically and create innovative solutions.
- Knowledge of person-centered practices
- Outstanding verbal and communication skills
- Consultative approach and ability to achieve rapport with a wide array of individuals
- You must have a driver’s license and insured vehicle which you can use for work-related travel.
EMPLOYMENT CONDITIONS:
- A Maryland driver’s license is required, along with an approved clean driving record.
- Must have a personal car for conducting agency business, with mileage reimbursement provided.
